How to Access Hospice
Programs & Services
Hospice welcomes
self-referrals. If you or someone you love is living
with a life-threatening illness, call Hospice at 632-5593– we
can help. Referrals to Hospice can also be made by your family
physician, your medical specialist, nurses, social workers, clergy and
Extra-Mural.
When You Call Us
When you call Hospice, you will be
directed to one of our two program coordinators. They are responsible
for assessing your needs, coordinating the delivery of our services,
supervising volunteers and serving as a supportive and empathetic
resource.
Our program coordinator will ask you
for your name, address and phone number as well as the name of your
family or attending physician. You will also be asked for general
medical condition, diagnosis and prognosis. Our program coordinator
will arrange a date and time to visit you to introduce herself, to
discuss your needs, review Hospice services and give you a complete
information package. Our program coordinators will also notify your
family or attending physician that you have requested our services.
